FamilyID/Forms

In order to be eligible to participate in sports, a prospective student-athlete must have a sports physical on file with the school. A sports physical is valid for 365 days and must be valid at the start of the sports season the student-athlete wants to participate in. Along with a sports physical, the student-athlete is also responsible for registering on FamilyID, which includes pertinent information in regards to the athlete and his/her parents. It will also include forms to read and sign off on, such as, the Concussion, Sudden Cardiac Death, and Steroid forms. The forms that must be printed and handed in to the Athletic Trainer are listed below. Please use the links below for FamilyID registration as well.
